Title:                             Licensed Practical Nurse (LPN)

 

Qualifications:

                                    Graduate from a State Board of Nursing approved school for

                                    Nursing.

                             Current CPR certification.

                             Such alternatives to the above qualifications as the State Board may find appropriate.

 

Reports to:                   RN and Building Principal

 

Job Goal:                      To assist the RN in a specialized practice of professional nursing that advances the well being, academic success and life-long achievement of students.

Performance Responsibilities:

  1. To practice LPN nursing care in accordance to the Nurse Practice Act written by the State Board of Nursing.
  2. To work under the scope of an LPN as written in the Nurse Practice Act for the state of Missouri
  3. To possess a basic understanding of state requirements pertaining to student health requirements and reporting.
  4. To carry out any nursing care procedures as delegated by the RN.
  5. To assist the RN in maximizing the quality of in class time by reducing the incidence of health related absenteeism.
  6. To assist in the assessment, planning, and evaluation of the health services component of the coordinated school health program.
  7. To assist the RN in providing nursing care to students at elementary, middle school and/or high school offices as needed.
  8. Reports any and all significant observations and/or parent concerns to the RN.
  9. Assurance that up-to-date cumulative health records on all students are maintained.
  10. Maintain confidentiality of student health records and information in accordance to HIPPA and FERPA guidelines.
  11. To assist the RN in carrying out health assessments ie. health, vision, hearing.
  12. To assist the RN in being a health care consultant for school personnel, students, and their parents/families.
  13. To assist the RN in the care of any student or staff member who has suffered an injury or emergency illness.
  14. Administer First Aid in accordance with established First Aid procedures as covered under Nurse Practice Act.
  15. Communicate with parents and/or guardians of students with health care needs.
  16. To participate in training for school-based staff to support children with healthcare needs ie. asthma, diabetes, epilepsy, etc.
  17. To assist the RN in implementing Board Policy on exclusion and readmission of student in connection with infections and contagious diseases.
  18. Maintain up-to-date current CPR certification.
  19. Other duties as assigned.

 

Duties:

1)      Health room management and triage

  1. Maintain cumulative health records.
  2. Maintain daily documentation of all students seen in the health room.
  3. Follow established policy and procedures for care of ill and injured students.
  4. Follow up on students with excessive absences for health reasons.
  5. Send out referral letters for health problems and screening failures after re-screening by registered nurse and referral decisions have been made.
  6. Follow up on referrals, maintain worksheet of referral status, document returned referral information.
  7. Implement fluoride rinse program and maintain required records on program.
  8. Assume responsibility for maintenance of health room supplies and equipment.
  9. Assist in identifying possible abuse and neglect and follow district reporting procedures.
  10. Ensure proper documentation for and administer daily/as needed (prn) medications.

 

2)      Screening Programs

  1. Screen all new entrants for height/weight, vision, hearing, and dental problems within one week of enrollment.
  2. Do initial screening for vision and hearing problems (in grades determined by RN).
  3. Screen students for pediculosis (head lice) as indicated by incidence.

 

3)      Special Health Care Needs

  1. Participate in development of emergency action plans for students needing emergency provisions.
  2. Implement individual health care plans, including special care procedures.
  3. Provide assessment of vision and hearing for students being evaluated for special education placement and report findings to RN.

 

4)      Education Activities

  1. Assist teachers in finding resources for health education.
  2. Participate in community outreach programs
  3. Assist school food service personnel to promote health food choices
  4. Assist school personnel with health needs, as indicated.

 

Evaluation Procedures During the School Year:

  1. Meet with nurse supervisor at least monthly regarding the school health program.
  2. Review medications and treatments at least weekly with nurse supervisor.
  3. Review health room documentation with nurse supervisor weekly or more often, if needed.

Physical Demands

While performing the duties of this position an employee is regularly required to sit, talk, and hear.  The employee is required to create and read handwritten or printed material.  The employee is occasionally required to stand, reach, and bend.

Conditions and Environment

The individual who holds this position will regularly work in a school and office environment that, at times, will be noisy and active.  Occasionally the individual will be required to work outdoors for short periods of time to perform nursing tasks that may arise at times such as during recess or as loading and unloading students from district transportation occurs.
